A Primary Care Network - often shortened as a PCN, is a group of GP practices based in a local community.
By working in a collaboration, practices offer more patient-centered approach and coordinated care for improved health outcomes.

Our PCN comprises of 14 GP practices, serving a population of over 86,000 patients.
We are covering wards of Harold Wood, Heaton and Gooshays' and are 1 of the 4 PCNs that cover the borough of Havering.
Our aim is to foster a culture of comprehensive, patient-centered care. We strive to enhance accessibility and quality of healthcare services, empowering individuals and communities to achieve optimal health outcomes.
Through collaboration and innovation, we aim to prioritise the well-being and satisfaction of our patients, ensuring they thrive in their journey toward better health and wellness.
PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home.
